      A patient presented with severe thoracolumbar kyph A patient presented with severe thoracolumbar kyphosis, resulting in an inability to stand upright and profound functional limitation. Preoperative assessment demonstrated significant sagittal imbalance and severe disability as measured by ODI.

After careful medical optimization, the patient underwent complex reconstruction from T4 to the pelvis, including a lumbar pedicle subtraction osteotomy (PSO) and transforaminal lumbar interbody fusion (TLIF) to restore alignment.

At one year:
✓ Upright, balanced posture restored
✓ Pain significantly improved
✓ ODI returned to the normal range

This case reflects a central principle in complex spine care: when deformity is severe, meaningful recovery depends not only on surgical technique, but on careful preparation, precise correction, and alignment-driven reconstruction.

For appropriately selected patients, restoring motion may represent an alternative to extending or repeating a fusion procedure.

      Progressive cervical deformity can become severely Progressive cervical deformity can become severely debilitating when patients lose the ability to maintain horizontal gaze and upright head posture, significantly impairing balance, mobility, and activities of daily living.

This 60-year-old patient presented with advanced “drop head” syndrome secondary to cervical extensor musculature weakness, resulting in progressive sagittal imbalance and substantial functional limitation.

The patient underwent a C2–T3 posterior cervicothoracic fusion with deformity correction and restoration of cervical alignment. At one-year follow-up, the patient demonstrated significant improvement in posture, horizontal gaze, and overall functional capacity.

Cases such as this highlight the role of complex spinal reconstruction in restoring not only structural alignment, but also independence, mobility, and quality of life in carefully selected patients.
